Do you Bulk Bill?
Yes, for Valid-
- Pensioner concession card holders
- Centrelink Health care card holders
- Children under 16yrs of age
- Department of Veterans affairs (DVA) card holders
Please Note: No Bulk Billing on Saturdays
What is your standard consultation fee?
For those not eligible for Bulk Billing, the standard consultation fee is $85.00, with a Medicare Rebate of $43.90.
Can I book a Telehealth appointment?
If you have been seen in person by a GP at our practice within the last 12months, then yes! Telehealth’s are available for simple urgent conditions, repeat scripts/referrals, medical certificates and some results. If you are unsure if your issue can be dealt with over the phone, then please call us on 02 47511 2211 and please speak to our friendly reception team for advice.
How does an FHC Telehealth consult work?
At Faulconbridge Health Centre, Telehealth appointments are conducted in a similar way to face-to-face consultations.
Appointments can be booked online or by calling the practice. Simply choose a time that suits you, and your doctor will contact you by phone at, or within a few minutes of, your scheduled appointment time.
To help ensure your consultation runs smoothly, please make sure that:
- Your phone is fully charged.
- The ringer and volume are turned on.
- You have your phone nearby and are available at your scheduled appointment time.
Do you do the Medicare Rebate on site?
YES! We use a TYRO eftpos machine that allow us to put your Medicare Rebate straight back into your nominated bank account (attached to your debit card). We can refund to Cheque or Savings accounts, but not Credit.
You CAN pay by credit and then use your cheque or savings account card to receive your rebate.
Do I need an appointment for a Doctor?
Yes, to see a doctor or a Nurse, you will need an appointment.
You can make an appointment over the phone by calling 4751 2211 or by using our online booking system HOTDOC.
Please note we are an appointment only practice- NO walk-ins.
Do I need an appointment for a blood test?
No, you can walk in with your pathology request form at any time.
Wise Pathology staff are on site Mon-Fri 8am-3:30pm, Sat 8am-1pm
If pathology is unavailable, you can take your request form to any pathology collection.
How do I know if i need a long appointment?
Standard appointment (15min) should be ample time for repeat scripts/referrals, simple results or medical certificates. Longer appointments will be needed for care plans/mental health care plans, driver’s licenses, diabetic reviews, work medicals, Cervical screening, complex or multiple medical conditions.
Should you require a longer appointment, please mention it to our reception team when booking your appointment. If booking online (HOTDOC), please click the appropriate appointment type.
Can I request repeat scripts and referrals over the phone?
Yes, you can request a telehealth appt for a repeat script/referral.
It is a Medicare requirement that a Patient has been seen by a GP within the last 12months in person to be bulk billed or to claim a Medicare rebate
Please note: if you have NOT been into our practice, within the last 12mths and still require a telehealth, this can be done, but it is privately billed, NO Medicare rebate available.
If you are a patient of ours, you can also request a Routine request for repeat scripts/referrals online through HOT DOC for a fee of $30-$40 which is not bulk billed. No appointment needed.
How do I get a Medical Certificate?
Medical certificates are issued ONLY during a consultation with a doctor either in person or via a telehealth appointment -Medical certificates cannot be back dated.
How do I get my diagnostic test results?
Your doctor will usually give you a time frame to book your follow-up appointment for results. If you do not already have a follow-up appointment, please call the practice after a week to book one in. Generally, this can be done in person or over the phone via telehealth. Results will only be given to patients once they have been discussed by a doctor.
How do I get to FHC?
Faulconbridge Health Centre (FHC) is located at 441 Great Western Highway, Faulconbridge NSW.
Our patient car park is accessed via Coomassie Avenue, behind the practice. We are the two blue buildings connected by a covered walkway, located next to the Faulconbridge Chinese Restaurant and directly opposite the local antiques store.
By Car
Travelling by car is the easiest way to reach the practice. For directions to our parking area, you can enter Stonehurst Place, Faulconbridge into your GPS or mapping app, which will direct you to the rear car park behind the shops.
Public Transport
A bus stop is located a short distance from the practice on the Great Western Highway, approximately a few hundred metres away.
Taxi Services
For patients requiring taxi transport, designated taxi pick-up and waiting areas are available:
- At the rear of the practice, adjacent to the disabled parking spaces.
- At the front of the blue building, under the covered area with the concrete forecourt.
Our reception team is happy to assist in contacting a taxi service of your choice following your appointment.
Accessibility
Disabled parking spaces are available in the rear car park, with convenient access to the practice.
FHC Patients are kindly asked not to park in the department of family and community services car park next door (437 great western highway)
